Role: Social Worker (LMSW, Temp-to-Permanent)
Location: Bronx, NY 10453
Facility Type: Mental Health/Supportive Housing Program
Pay: $31.00 – 34.00/hour (during contract, $70k salary upon permanency)
Essential Duties:
· Under the supervision of the Director of Social Service, responsible for client case management and coordination of client services.
· Work within a crisis driven environment with a knowledge base of residential services for individuals with a history of homelessness, substance use/addiction, IPV, trauma, medical & mental health conditions, and co-occurring disorders.
· Able to work from a person-centered approach, trauma-informed care, strengths-based perspective, or solutions-focused approach.
· Knowledge about states of change and motivational interviewing skills.
· Assist clients with specific stressors associated with daily living and living with certain medical conditions, mental illnesses, or substance use disorders.
· Confer and consult with professional and technical personnel in implementing a multidisciplinary approach to client care and well-being.
· Responsible for the completion of psychosocial assessments of complex clients impacted by multiple systems.
· Assess client needs and facilitate appropriate linkages for clients to community-based services.
· Provide education to other members of the team around clients’ psychosocial stressors and needs to ensure positive outcomes for tenants.
· Serve as agency/program liaison to community agencies and/or groups.
· Provide clear written reports that capture key findings to include provider areas of strength, and recommendations for improvement.
· Attend ongoing professional development by agency and other funders as needed.
Requirements:
· Active NYS LMSW or LCSW licensure required.
· 2+ years of experience working with mental illness, substance abuse, HIV/AIDS, and/or homelessness is required.
· Experience in assessment, service planning, and community referrals preferred.
· Knowledge of supportive housing, HIV/AIDS, and substance abuse required.
· Bilingual English/Spanish a plus.
· Ability to work as a member of an interdisciplinary team.
· Ability to work flexible schedule when needed consisting of evening or weekend hours.
· Comply with Employee Policies, Procedures, & Protocols, Corporate Compliance, and NYS Dept. of Social Services rules, regulations, and statutes.
· Working knowledge of MS office (Word, Excel, PowerPoint) and email.
