We are seeking a dedicated and compassionate Occupational Therapist to join our home health agency. The ideal candidate will provide high-quality therapy services to clients in their homes, promoting independence and enhancing their quality of life.
This position requires travel to various locations and offers a schedule of Monday to Friday, 8 AM to 5 PM, with a minimum of 35 visits per week.
Key Responsibilities:
– Conduct comprehensive assessments of clients' needs and develop individualized treatment plans.
– Implement therapeutic interventions to improve clients' physical, emotional, and cognitive functioning.
– Collaborate with interdisciplinary teams to ensure coordinated care for clients.
– Educate clients and their families on treatment plans and strategies for daily living.
– Maintain accurate and timely documentation of client progress and treatment outcomes.
– Adhere to all regulatory and compliance standards in home health care.

Requirements:
– Master's or Doctorate degree in Occupational Therapy from an accredited program.
– Current state licensure as an Occupational Therapist.
– Previous experience in a home health setting is preferred.

– Valid driver's license and reliable transportation for travel to client locations.
