SUMMARY: The Supervisor of Care Continuum works in collaboration with the multidisciplinary team to develop and coordinate comprehensive, timely, safe, cost effective, and quality healthcare services for service line patients throughout the care continuum. EXPERIENCE: Minimum of two years of bedside nursing experience. Preferred- Case Management experience. Working knowledge of alternative payment methodology, insurance coverage, postacute providers as well as community resources. SPECIAL SKILLS: Ability to gather, track, and analyze data and outcomes of program initiatives and reimbursement models. Excellent verbal and written communication skills required. OTHER: Demonstrates strong organizational, critical thinking, analytical, and leadership skills as well as the ability to work in teams and prioritize patient needs. LICENSE/CERT: Required: RN (Registered Nurse) State of Missouri EDUCATION: Required: Bachelors – Nursing
