We’re Hiring!. Nurse Practitioner – Mobile Primary Care (SW Portland Metro)
Are you a Nurse Practitioner who loves the idea of practicing independently while building meaningful relationships with your patients?.
We’re a mobile primary care clinic looking for someone like you to join our team!
You’d be working with older adults in assisted living facilities and memory care communities around Beaverton and Southwest Portland. With the help of your own Medical Assistant (MA), you’ll visit each community at least once a month, providing compassionate, patient-centered care at a pace that works for you.

What You’ll Do
This role is all about building relationships and delivering consistent, high-quality care.
You will:
– Visit your assigned communities monthly (or more, as needed)
– Round on your assigned patients (5+ patients per community) providing high quality medical care in their homes
– Work closely with patients, patient families, and facility staff to coordinate care
– Create and manage care plans tailored to your patients’ needs
– Document your visits in collaboration with your Medical Assistant
Who We’re Looking For
We’re looking for someone who is:
– A licensed Nurse Practitioner in Oregon (required)
– Proficient in EHR, Telehealth, and e-Fax usage
– Willing to travel around the SW Portland, Tigard, Aloha, and Beaverton areas with reliable transportation
– Passionate about working with older adults
– Experienced in primary care or geriatrics (or interested in learning!)
– Comfortable working independently and managing their own schedule in conjunction with the home office and medical assistant
